ProMedica Hospice and Palliative Care, formerly Heartland Hospice is part of the HCR ManorCare family, a leading provider of home health care, hospice care, skilled nursing, memory care and post-acute care. We are searching for an experienced professional to be a part of an exciting opportunity to grow a palliative care and hospice admissions team in Buffalo Grove, IL.
The Admissions Director is an internal sales position that is responsible for coordinating the referral to admission process for palliative and hospice care. Supervises admission team and procedures to ensure that eligible patients receive care, barriers to admissions are addressed to resolution, patient and agency growth and profitability meets or exceeds standards.

Educational Requirements
RN Preferred. Bachelor Degree with considerable experience in a comparable position will be considered in lieu of nursing license.
Position Requirements
• Demonstrated knowledge of customer service
• Minimum 2 years experience in home health, hospice, or infusion
• Demonstrated knowledge of payer sources and case management review
