Nights 5pm-3am (4) 10-hr shift
As a RN Case Manager, your ability to manage client care with specific knowledge and experience in bedside care, symptom management, crisis intervention and family intervention are feature assets for this high-profile nursing position. In addition to excellent written/verbal communication, problem solving and decision making abilities, our candidate has a demonstrated experience developing and maintaining rapport with patients and families, and the ability to work well with an interdisciplinary team.

4635 - ProMedica Hospice - Portsmouth Office - Serving Southern OhioQualifications
A minimum of 6 months nursing experience within the past 3 years unless state regulations differ. Travel is necessary on a daily basis.
Job Specific Details:Nights 5pm-3am (4) 10-hr shift
- Manages a Patient and family caseload
- Manages, plans, coordinates and delivers nursing care to patients and families.
- Supervises and documents home health aide/licensed practical nurse plans of care and documents supervisory visits.
- Participates in on-call coverage.
- Participates in regularly scheduled interdisciplinary team
- Applies the policies and procedures of the agency and the rules and regulations of Federal and State regulatory agencies and other certifying agencies in providing care.
- Assesses & evaluates Patient?s level of care, needs and requirements.
- Participates in agency quality assessment performance improvement (QAPI) programs.
- Assesses the Patient?s response to the Plan of Care and recommends changes as needed.
- Provide counseling to Patient/family regarding disease process, end of life and ethical issues.
- Willingly accepts direction from Director of Professional Services/PCC.

Educational Requirements
Currently licensed as a Registered Nurse (RN) in the state and in good standing with the Board in which he/she will practice.
