Masters Degree in Social Work from an accredited institution.May require licensure or certificate as required by the state, and in good standing. (if applicable)
Position Requirements
Prefer one year experience in a health care setting. 100% travel necessary on a daily basis. Demonstrate problem solving and decision making skills. Demonstrated ability to work will with an interdisciplinary team. Excellent communication skills. Ability to develop and maintain rapport with the client and family.
Job Specific Details:- Assesses emotional factors related to terminal illness.
- Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect.
- Assesses environmental resources and obstacles
- Participates in the development and revision of the plan of care.
- Short-term individual counseling, Crisis intervention
- Identifies family dynamics and communication patterns.
- Identifies and utilizes appropriate community resources
- Participates in in-service programs
