$1,500 Sign On Bonus.  ProMedica Senior Care, formerly HCR ManorCare, is one of the leading providers of Hospice, Home Care, Skilled Nursing, Memory Care, Palliative Care, and Assisted and Independent Living. Heartland, ManorCare, and Arden Courts are now part of the ProMedica family of services. The home health and hospice, skilled nursing, and rehabilitation services you know us for are now part of an integrated health and well being organization.

ProMedica Home Health Care and Hospice is searching for an experienced customer service professional to fill the role as an Admissions Coordinator.  The Admission Coordinator is an internal customer service position responsible for coordinating the referral-to-admission process for home health, hospice, and palliative care patients. They provide administrative support and ensure that all eligible patients receive care, barriers to admissions are addressed and resolved and agency(ies) growth and profitability is maximized.  

We have a 10:30am-7pm and 8am-4:30pm Monday - Friday shift available.  Limited after hours on call required.  On call is done remotely but weekday hours are located in our Milwaukee office off of Innovation Drive.

Educational Requirements

Associate Degree in related field required. Requisite experience may be considered in lieu of Associate Degree. Bachelor's degree preferred.

Position Requirements

Demonstrated customer service experience Health care experience required. Travel is minimal but necessary on occasion as directed by sales manager.
