ProMedica Hospice (formerly Heartland Hospice), a leading provider of hospice care, is currently recruiting for a RN Nurse Liaison/Care Transition Nurse/Nurse Navigator. We are seeking an experienced hospice, home health, disease management, case management, or oncology nurse to fill the role. This is primarily a Monday – Friday position.
The RN Liaison is responsible for providing support to transition a patient smoothly between care settings / levels of care. They work collaboratively with the patient, family, facility discharge planning team and Heartland team for the care transition of patients choosing hospice services. The responsibilities include explaining the benefits of hospice, assessing eligibility, and completing sign on to the Hospice benefit. The RN Liaison also partners with the Account Liaisons to educate, develop and maintain referral source relationships.

Minimum of two years of experience delivering home health care, IV care or hospice services.
Demonstrated knowledge of payer sources.
Demonstrated knowledge of the referral source types and community resources.
Registered Nurse required. Licensed and in good standing in the state in which he/she will practice.
