Heartland Home Health Care and Hospice, a subsidiary of HCR ManorCare/ProMedica, is currently searching for a full-time, .8 FTE, hospice and home health Triage Nurse. As a triage nurse you will be responsible for handling incoming calls after hours, doing clinical assessments over the phone, providing clinical expertise and coordinating visits with the on-call nurse(s) for select agencies as needed/appropriate. This is a position working from home and has a schedule that includes working every Friday 5pm-8am, Saturday and Sunday 8am-5pm or 5pm-8am. Must be able to work either shift as hours will vary depending on need. Currently 2 holidays/year 8am-5pm or 5pm-8am (shift times subject to change). This role will also be a backup for call offs. This position will triage for several home health and hospice locations within WI, MN, IL, IN and in the future IA. RN licensure in these states is required. May consider experienced candidates with only a WI or IL license to start with the understanding that they will be required to obtain remaining licenses shortly after starting employment with Heartland.
The Triage Nurse is responsible for providing nursing triage services. Functions to assess the severity of the patient’s health concerns, implementing nursing interventions and arranges skilled nursing or support staff visits as indicated on an individual basis.
Ability to manage Patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention and family intervention.
Ability to work well with an interdisciplinary team.

Demonstrate problem solving and decision-making abilities.
Ability to develop and maintain rapport with the Patient and family.
Capacity to function independently with licensed scope of practice.
Must have the ability to handle stressful emergency situations calmly and effectively.
Minimum of 2 years of hospice experience preferred.

Currently licensed as a Registered Nurse (RN) in the state and in good standing with the Board in which he/she will practice.
A minimum of 6 months institutional nursing experience within the past 3 years, unless state regulations differ. Travel is necessary on a daily basis.Job Specific Details:
